#2 - If you work for a company with a "Matching Gift Program" please get the form and complete Part A, send both front and back to me and let me know you are sending a form.

If you would like to donate to the ACVA via   

Place the quantity in the box to the right of the amount you would like to donate (1, 2, 3, etc.). For instance, if you would like to donate $1,000, enter the number 1 in the quantity box after the $1,000 label. If you would like to donate $2,000, enter the number 2 in the quantity box after the $1,000 label.

For those of you who are not used to using a credit card on line, you ARE required to enter your security code in the "card security code" field or the transaction will not process. This is for your protection! The security code is a 3 character number on the back of your card. If you have any problems, contact me.

A FEW HAVE REPORTED THEIR TRANSACTION FAILED DUE TO NO ENTRY IN THE "COMPANY" FIELD - AND THERE ISN'T ANY "COMPANY" FIELD. THIS WAS A PROBLEM AT OUR PROVIDER'S END AND HAS BEEN FIXED (ACCORDING TO THEM) AS OF 11:30 AM EST, 02/29/08. WE ARE SORRY FOR ANY INCONVIENCE THIS MAY HAVE CAUSED.
